Full Job Description

Job Title: Case Manager

 

Department: Case Management

 

Reports To: Supervising Case Manager/Administrator

 

FLSA Status: Hourly

 

SUMMARY

 

Shall provide case management services as outlined in the State Department of Health regulations. All services provided, shall support the resident in maintaining independent functioning, personal choice and enhance the quality of life and well-being of each resident.

 

Responsible/Accountable for the accuracy and validity of all recorded documentation pertaining to your job description.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

 

Complete initial resident evaluation of the case needs of the resident within thirty days after admission. This evaluation will be updated every twelve months.

 

Assist resident with adjustment to the facility by:

 

  1. Reviewing and providing explanation of the admission and financial agreement in conjunction with their admission.

 

  1. Review and document that the Bill of Rights, health care proxy, grievance procedure and gratuity policies have been explained.

 

  1. Orient resident to the facility and assist personal care with orientating resident to the daily routines.

 

  1. Provide resident with encouragement to participate in facility and community activities of choice.

 

  1. Assist resident with making application and arranging for services from public and private sources of income, health, mental health, and social services.

 

Assist resident in making application when in need of alternative living arrangements if the resident no longer meets the admission criteria.

 

  1. Clarify discharge placement process with resident and family.

 

  1. Comply with Department of Health documentation requirements for resident discharge to higher level of care.

 

  1. Execute sound discharge and transfer plans.

 

Coordinate resident discharge with facility departments; admissions, personal care, housekeeping, and maintenance.

 

Assist resident in obtaining and maintaining a primary physician or source of medical care of choice when necessary.

 

Assist personal care staff in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident health or mental health including but not limited to:

 

  1. Health and mental health services

 

  1. Dental services

 

  1. Rehabilitation services

 

Responsible to remain informed and communicate with other departments as well as resident representative any significant issues regarding the residents care and supervision needs.

 

Review daily communication notes of assigned resident caseload and provide assistance with coordinating services needed.

 

Communicate openly with supervisor significant information regarding the day-to-day operations of the case management department.

 

Provide resident with explanation of facility grievance procedures.

 

Coordinate with Director of Personal Care, admissions, re-admissions, transfers, or resident discharges.

 

Keep all residents informed of changes relevant to Medicare, SSI, Medicaid, secondary insurances, and assist resident with resolving any billing issues.

 

Provide resident with explanation of Personal Allowance Account function.

 

Document daily interaction in case management progress notes.

 

Assure resident rights are protected and they receive confidential, fair, courteous, and respectful care at all times.

 

Calculate and document the resident daily census.

 

Coordinate and complete pre-screen interviews and resident admissions in the absence of the Admissions Director.

 

Complete all necessary paperwork for new admissions.

 

Submit updated mental health tracking forms monthly to Supervisor and Administrator.

 

Other duties as assigned by Administration.

 

Responsible for providing accurate information pertaining to the agency to community agencies and institutions, as well as the general public.

 

Provide informative tours to referrals.

 

Ensure that all filing is kept current.

 

Required to attend the following meetings:

 

  1. Monthly Resident Council

 

  1. Food Committee

 

  1. Welcome/Admissions Committee

 

  1. Weekly Case Management Departmental meeting

 

  1. Weekly Interdisciplinary Department meeting

 

  1. Community Agency meetings as assigned

 

  1. Monthly inservice

 

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.

 

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

Bachelor’s degree or Masters degree from an accredited college or university with a major in human service delivery, and 1 year of full-time experience in the provisions of services to an adult care population.

 

OR

 

Associates degree from an accredited college or university with major work in human service delivery, and 3 years full-time experience in the provision of services to an adult care population.

 

LANGUAGE SKILLS

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of that organization.

 

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent to draw and interpret bar graphs.

 

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or scheduled form.
