Within our Residential Division:

Residence Manager – Responsible for the day-to-day operation and management of the residence.Provides supervision and assistance to residence staff and ensures implementation of individualized programming for each individual who resides at the residence.Assists the Residence Coordinator in the development, monitoring and updates of residential habilitation plans.Provides direct input in the assessment, as well as, the design and implementation of individual behavior management programs.

Residence Coordinator -To effectively manage all phases of the day-to-day operation of his/her assigned facilities. Maintain and/or enhance the provision of quality services to individuals receiving services by monitoring and implementing facility regulations, policies and procedures, and standards of good practice. Primary function is to coordinate all services to meet individual needs of individuals on assigned caseload, including a willingness and ability to look at new ideas and philosophies to create a quality of programming for each individual. In addition, is responsible for ensuring adherence to quality assurance standards within homes.  

LPN – To provide directly and/or to ensure through observation and monitoring, that the health care services necessary to meet the needs of the individuals in an assigned residence are provided in a timely and effective manner. To assist in the provision of personal care and direct support services for the individuals in an assigned residence

RN – To serve as an active member of the Interdisciplinary Team; To promote and enhance person-centered outcomes; To serve as medical coordinator/advisor/liaison on behalf of consumers medical needs/interests in conjunction with family member wishes and community provider recommendations; To ensure consistent compliance with established treatment plans; To serve as a mentor to LPNs and recent graduates; To promote a life of hope, dignity, and purposeful living for all.


Within our Clinical Division

Primary Care Practice Manager – Responsible for the smooth, efficient and cost-effective operation of assigned clinical programs that serve a diverse patient population. In conjunction with and/or under the direction of the Director, is responsible for all activities that improve performance on the triple aim of improved individual and population health, improved patient experience, and reduced cost. To guide the day to day aspects of satisfactory patient visits, workflow efficiency, quality of care and health outcomes. Ensuring adherence with all regulatory compliance, quality assurance plans and fiscal management oversight.

Revenue Cycle Administrator – Responsible for the overall management of Clinic billing, revenue recognition and collection operations, including planning, directing, supervising, coordinating and evaluating the day-to-day operations ensuring that processes and data meet regulatory requirements and policies. The Manager works to standardize and centralize all billing functions to increase efficiency and ensure the Agency maximizes revenue for all services provided (i.e., self-pay, Medicaid, Medicare, other government programs and other payers.) Continually analyzes billing and payment statuses and prepares and presents detailed reports, summaries and analysis to Senior Management along with recommendations for improvement. It is the goal of this position to have 95% of claims submitted to payers successfully result in payments on the first submission.
